Since name changes require proof of ID, we shouldn't be allowing employees to change their name. Also, we just found a situation where someone changed their name and never told anyone - which affects all of our other systems.

Absolutely agree! Name and phone number changes affect all of our systems and should only be done by administrators. And we also require proof of ID for name changes.
